Program:
Full Service Partnership - TAY - SacramentoJOB SUMMARY: This position assists and supports TAY, their family members, and other FSP team members in operationalizing strength-based, flexible, recovery-oriented services through coordination of services and direct provision of rehabilitation and case management services.
MINIMUM QUALIFICATIONS
Education and Experience
- Bachelor's degree in a social science field plus at least one (1) year of experience working in a behavioral health field.
- Direct experience with receiving services from the public mental health system, alcohol and drug services, child welfare, and/or juvenile justice system is preferred.
License or Certification
- Valid California Driver's License.
DUTIES AND RESPONSIBILITIES
Essential Duties
- Uses a “whatever it takes” approach to provide services in various settings, including homes, schools, and community and office settings.
- Uses personal vehicle to drive to and from offsite locations to provide services. May be required to transport participants and/or family members in personal vehicle. Incorporates Transition to Independence Process (TIP) into services. Provides services, including rehabilitation, collateral, behavior management, school support, crisis intervention, and skill development, to TAY and their families, as deemed necessary and appropriate through the assessment process and in collaboration with the team of providers.
- Provides Outreach and Engagement to TAY and families within the community. Provides case management and linkage services to TAY and their families.
- Collaborates with other providers assigned to each participant to determine appropriate plan of care for each participant.
- Documents all services provided in a clear and concise manner within the Electronic Health Record within the timelines and framework established by Capital Star.
- Participates in treatment team meetings. Assists with follow-up on referrals made into the program.
