What You Bring To SBHG (qualifications)
Education / Experience
- HS degree and six (6) years of direct service experience in the Mental Health field OR
- Associates degree and four (4) years of direct service experience in the Mental Health field OR
- Bachelor’s degree and two (2) years of experience working in the mental health field
- Valid California Driver's License
- Meets Agency driver eligibility criteria

The Case Manager will provide services that may include case management/linkage, assessments, screenings, treatment planning/monitoring, and rehabilitation services to the clients and their families.
Division/Program Overview
- Supporting individuals with mental health, family life, employment, education, living situation, personal effectiveness, and community life functioning.
- Sessions are in the community, clinic, home and some telehealth
- Working with individuals ages 6-15 (Child) and 16-25 (TAY)
- Utilizing a strength-based approach and working together as a multidisciplinary team
- We provide services to individuals and families throughout Santa Clara County
