What You Bring To SBHG
- High School diploma or equivalent (e.g. GED, HiSET, etc.) required plus valid nursing license required OR
- Master's degree in social work, marriage & family therapy, or psychology from an accredited college or university required plus valid clinical license required.
- Two (2) years direct service experience with severely emotionally disturbed adolescents, in an inpatient health care setting preferred. OR
- One (1) year service experience medically treating the mentally ill in an inpatient setting preferred.
- Valid California LVN, LPT, AMFT, APCC, ACSW, LMFT, LPCC, or LCSW required.
- Provides on-going case management and follow through with clients on the Central Star waiting list.
- Assures all legal admission requirements are obtained for each new client including involuntary 5150, voluntary by youth (Roger S), conservatorship, or clients signed in by parents/legal guardians.
- Coordinates the receipt of client information with all relevant information.
- Completes Intake Reports and Denial Logs that chronicle referrals, admissions, and referrals that do not result in admissions. Inputs referral information into EMR or provide the appropriate information for Nursing or PHF Clerk/Reception staff to enter the client in the EMR intake.
- Before or upon admission: Assures initial authorization of payment from payment source (authorization from private insurance company, etc.). Obtains Medi-Cal card. Obtains identification documents.
- Completes Assessment before or upon admission—including strengths, dates for hospital and other placements, meds and dose info, cultural factors, etc.
- etc.
- 24/7 inpatient involuntary program
- 16-bed facility for acute youth (ages 12-17)
- Intensive treatment programs with individualized care plans
- Available for contracting Central Valley counties
- Moves clients through five phases: admission; engagement; learning; stabilization and linkage to resources and aftercare
