Clinical Liaison / Clinical Consultant (RN or MSW) – California Pay Rate: $50.00–$53.00/hrPosition Type: Special Projects – 12-Month W2 Contract(renewal based on performance, project continuation, and health plan needs)Schedule: 9:00 AM – 5:30 PM PST, Monday–FridayModel: Remote-First (with limited field visits as needed)Star Nursing Services is hiring a Clinical Liaison / Clinical Consultant (RN or MSW) to support statewide CalAIM programs, including the Assisted Living Waiver (ALW), Enhanced Care Management (ECM), and Community Supports.This role focuses on remote clinical coordination, assessments, review of referrals, transition planning, and policy-driven program oversight.Field work is minimal, but the Consultant must be available for occasional in-person assessments or partner site visits when required. What You’ll DoCalAIM, ECM, ALW & Community Supports• Educate Medi-Cal members, families, providers, and facility staff on:– Assisted Living Waiver (ALW)– Enhanced Care Management (ECM)– Community Supports• Complete assessments (remote or in-person as needed)• Review member eligibility, clinical data, and transition needs• Identify resources and supports related to housing, health, and long-term care• Ensure all documentation follows DHCS and MCP standards Hospital & SNF Coordination• Liaise with discharge planners, social workers, and case managers• Manage high-volume referrals and ensure efficient, timely processing• Assist in transition-of-care planning for ALW and CalAIM programs• Gather clinical/psychosocial data needed for application review Care Management, Assessments & Planning• Conduct remote assessments, care plan review, and follow-up• Support ALW, ECM, and Community Supports transitions• Facilitate access to community-based programs• Participate in interdisciplinary care coordination when requested Quality, Compliance & DHCS Requirements• Ensure work adheres to DHCS policies, procedures, and program requirements• Review assessments, care plans, and EMR documentation for compliance• Support audit readiness, CAP implementation, and quality improvement needs• Maintain accurate progress notes and electronic records• Respond to DHCS inquiries, MCP questions, and program oversight items Referral Source Relations & Program Awareness• Serve as an educational resource for hospitals, SNFs, ALFs, and partners• Promote understanding of ALW and CalAIM benefits• Track referral sources and prepare trend reports Remote-First Expectations• Role is primarily remote• In-person visits may be required periodically• Travel, if needed, will be local and reasonable Required Qualifications• Care/Case Management experience required• RN or MSW required• 2 years experience in:– Case management– Acute/sub-acute care– SNF or long-term care– Hospital discharge planning– ALW / CalAIM experience preferred• Strong understanding of:– Medi-Cal– DHCS policies, programs, and procedures– Assisted Living Waiver– Enhanced Care Management– Community Supports• Ability to manage high-volume work• Excellent communication and rapport-building skills• Strong organizational and documentation skills Transportation (Occasional Field Needs)• Unrestricted California driver’s license• Valid vehicle insurance• Current registration• Reliable vehicle(used only for limited field visits if assigned) Technology Requirements• Intermediate skill with Microsoft SharePoint, Excel, Word, Teams• Adobe applications (PDF editing, forms, signatures)• EMR experience (CalAIM-specific preferred) Minimum PC SpecificationsWindows 10 or higher8GB RAM minimum (16GB preferred)Intel i5 / AMD Ryzen 5 or higher256GB SSD or largerChromebooks not allowedScreenshot of system specs required during the interview Education• RN or MSW required• Additional training in case management, gerontology, public health, or social services preferred Why This Role MattersThis position ensures members receive timely coordination, accurate program guidance, and high-quality clinical review.
