This is a one-year contract with renewal consideration based on performance and program continuation. What You’ll Do Lead Care Managers delivering Enhanced Care Management (ECM) and Community SupportsComplete frequent in-person home visits, assessments, care plan updates, and transitions of careSupport escalations and collaboration with health plan partnersTrain new staff on CalAIM workflows, county resources, and documentationReview EMR documentation for accuracy and MCP alignmentSupport audit readiness, findings, and CAP implementationPartner with ECM clinical teams, community agencies, and housing providers Important – Field Expectations Multiple in-person visits every weekDaily travel across Yolo, Yuba, Sutter, Solano, or Colusa countiesRemote work is limited Required QualificationsCare/Case Management experience requiredMinimum 1 year in CalAIM ECM/CS, case management, public health, or related fieldsStrong understanding of Medi-Cal, Enhanced Care Management (ECM), Community Supports, authorizations, and managed care requirementsExperience with appeals, grievances, audits, CAP findings, and corrective action plansKnowledge of County resourcesStrong communication and leadership skills2 character referencesMust reside in Yolo, Yuba, Sutter, Solano, or Colusa counties Transportation Requirements• Unrestricted active California driver’s license• Valid auto insurance• Current vehicle registration• Reliable vehicle for county-wide travel Technology Requirements• Intermediate experience with:– Microsoft SharePoint– Microsoft Excel– Microsoft Word– Microsoft Teams– Adobe (PDF editing, forms, e-sign tools)– EMRs for CalAIM documentation Minimum PC SpecificationsWindows 10 or higherMinimum 8GB RAM (16GB recommended)Intel i5 / AMD Ryzen 5 or higher256GB SSD or largerChromebooks not acceptedScreenshot of computer specs required during interview EducationAA/BA, or equivalent experience in case management, housing navigation, social services, public health, or related CalAIM functions Why This Role MattersMembers face significant housing, financial, and health barriers.
