Mental Health Practitioner (MHP)

We are seeking a compassionate and dedicated Mental Health Practitioner (MHP) to join our Adult Rehabilitative Mental Health Services (ARMHS) team in Minnesota. The ideal candidate will provide rehabilitative mental health services to adults with serious and persistent mental illness, focusing on improving independent living skills, emotional regulation, symptom management, and community integration.

This role involves working with adults in their homes and community settings using person-centered, recovery-oriented, and evidence-based approaches. The practitioner will support clients in achieving treatment plan goals through skills training, support services, and coordination of care. Familiarity with mental health documentation and 10 diagnostic coding is preferred. Responsibilities

• Conduct client intake interviews and functional assessments in accordance with Minnesota DHS ARMHS requirements.
• Assist in developing and implementing individualized treatment plans (ITPs) based on diagnostic assessments and identified rehabilitation goals.
• Provide ARMHS services focused on:
• Symptom management
• Coping and emotional regulation skills
• Daily living and independent living skills
• Social skills and community integration
• Deliver rehabilitative interventions using evidence-based practices such as cognitive behavioral strategies, psychoeducation, and skills training.
• Support clients in managing mental health symptoms related to diagnoses such as depression, bipolar disorder, schizophrenia, PTSD, and anxiety disorders.
• Respond to crisis situations using appropriate de-escalation and crisis intervention techniques to ensure client safety.
• Coordinate care with supervisors, case managers, psychiatrists, and other service providers to support continuity of care.
• Maintain accurate and timely documentation in compliance with DHS, HIPAA, and agency standards.
• Document services using appropriate medical terminology and diagnostic codes for service authorization and billing purposes.
• Participate in supervision, team meetings, and ongoing training to ensure service quality and professional development.

Skills

• Knowledge of rehabilitative mental health practices for adults with serious and persistent mental illness.
• Experience providing skills-based interventions such as symptom management, coping strategies, and independent living skills training.
• Strong understanding of crisis intervention and safety planning techniques.
• Familiarity with ICD-10 diagnostic coding and mental health documentation standards.
• Ability to work with adults experiencing trauma, depression, anxiety, bipolar disorder, schizophrenia, or co-occurring conditions.
• Excellent communication, problem-solving, and interpersonal skills.
• Ability to work independently in community settings while collaborating with a multidisciplinary treatment team.
• Commitment to maintaining client confidentiality and compliance with HIPAA and Minnesota DHS regulations.

Pay: $25.00 - $26.00 per hour
