JOB SUMMARY

The Community Program Specialist is responsible for coordinating, aligning, and optimizing resources that support Stanford Medicine Children's Health community-based children's health initiatives. This position ensures that community health programs are efficiently administered, data-drived, and effectively integrated with hospital and partner resources to improve access, equity and outcomes for children and families. The Specialist works collaboratively with internal stakeholders, external partners and community organizations to strengthen program infrastructure, streamline processes, and enhance the impact of our most complex patients.

Essential Functions

Employees must abide by all Joint Commission Requirements including but not limited to sensitivity to cultural diversity, patient care, patient rights and ethical treatment, safety and security of physical environments, emergency management, teamwork, respect for others, participation in ongoing education and training, communication and adherence to safety and quality programs, sustaining compliance with National Patient Safety Goals, and licensure and health screenings.

Must perform all duties and responsibilities in accordance with the hospital's policies and procedures, including its Service Standards and its Code of Conduct.

  • Coordinate the planning, implementation, and evaluation of community health programs focused on children's health and wellness.
  • Monitor program workflows, budgets, and resources to ensure efficient and effective operations.
  • Develop and maintain resource tracking tools, databases, and documentation to support performance improvement and reporting.
  • Build and sustain collaborative relationships with community organizations to strengthen referral pathways and support services.
  • Assist with identifying and securing external funding opportunities such as grants, sponsorships, and community benefit investments.
  • Attend specialty departmental meetings to align patient/family needs with needed services and educate staff on opportunities for discovery.
  • Maintain consistent verbal and written communication with program managers/Directors, team members as well as external stakeholders.
  • Escalate any issues to appropriate stakeholders for resolution and track until completion.
  • Serve as the escalation point for referral-related issues, maintain issue logs, and provide timely resolution. Coordinate issues with external departments as needed for resolution.

Qualifications

MINIMUM QUALIFICATIONS

Any combination of education and experience that would likely provide the required knowledge, skills and abilities as well as possession of any required licenses or certifications is qualifying.

  • Education: Bachelor's degree in a work-related discipline/field from an accredited college or university.
  • Experience: Three (3) year of progressively responsible and directly related work experience.
  • License/Certification: none.

KNOWLEDGE

These are the observable and measurable attributes and skills required to perform successfully the essential functions of the job and are generally demonstrated through qualifying experience, education or licensure/certification.

  • Ability to communicate effectively, both orally and in writing.
  • Ability to plan, organize, develop tactical plans and set priorities.
  • Ability to work effectively both as a team player and leader.
  • Knowledge of computer systems and software used in functional area.
  • Knowledge of marketing and community relations, assessment methodologies, practices of planning, developing and facilitating community programs and measuring results.
  • Knowledge of process improvement principles and design principles.
  • Strong relationship management and communication skills are essential.
  • Ability to interpret data sets, identity trends and make data driven recommendations.

Pay Range

Compensation is based on the level and requirements of the role.

Salary within our ranges may also be determined by your education, experience, knowledge, skills, location, and abilities, as required by the role, as well as internal equity and alignment with market data.

Typically, new team members join at the minimum to mid salary range.

Minimum to Midpoint Range (Hourly): $35.48 to $47.07
