Position: Fitness Project Manager (Director)

Department: Diabetes

Position Summary:

The Diabetes Prevention Director provides leadership, direction, and oversight for the design, coordination, implementation, and evaluation of diabetes prevention, care, and management programs. This role ensures all activities align with funding requirements, organizational objectives, and Tribal community health needs. The Director works collaboratively with staff, patients, families, and community partners to reduce diabetes prevalence, promote wellness, and enhance the quality of life for individuals affected by or at risk of diabetes.

Key Responsibilities:

Program Improvement & Advocacy

  • Enhance, improve, and promote comprehensive care for diabetic clients to support quality-of-life improvements and medical outcomes.
  • Monitors case management services; ensuring data is collected and recorded for appropriate reports.
  • maintain current knowledge of diabetes prevention, management, and other relevant health issues.
  • Participate in the development of marketing and promotional strategies, write grant proposals, and seek funding opportunities to sustain and expand diabetes-related programs.
  • Oversees comprehensive program and departmental assessments, using data-driven evaluation methods to guide strategic planning and continuous improvement of diabetes prevention services.
    Partners with other Tribal Programs including Tribal Health to support, collaborate, and improve outreach and program efficiency.

Education & Community Engagement

  • Provide extensive information and education on diabetes prevention, care, and management to the public, referred patients (including schools/children), staff, and community groups upon request.
  • Plan and coordinate with staff and community partners to develop patient education and health promotion activities, ensuring effective use of facilities and resources.
  • Review and maintain updated health status data and assess local health needs—particularly among Standing Rock Tribal members, at risk individuals, public event attendees—by engaging patients and families through surveys and/or focus groups to identify health problems and develop culturally appropriate educational approaches and materials.
  • Collaborate and advocate with Tribal/Community health programs to promote outreach, inclusion, and education initiatives supporting individuals with or at risk for diabetes.
  • Provide leadership and collaboration in patient education programs through joint planning, problem solving, innovation, and evaluation of outcomes.
    Regular travel to Tribal Communities and schools to coordinate activities, sessions,

Fiscal/Contract Management

  • Provides oversight and management in all areas of the program fiscal management including budgets, reconciliation, budget modification, journal vouchers, drawdowns, budget narratives, and financial program reports.
  • Ensure cost effective operations and efficient use of organizational and program resources that aligns with all applicable SRST Policies.
  • Completes required monitoring and quarterly reports on program activities and expenditures for submission to the funding agency, oversight committees, and organizational leadership.
    Oversees compliance for all program grants and contracts, including monitoring performance metrics, preparing and submitting required compliance reports, and ensuring adherence to all contractual terms and obligations.

Program Leadership & Compliance

  • Provide direction, coordination, implementation, and evaluation of the Diabetes Program in accordance with funding requirements, organizational policies, and public health standards.
  • Review, evaluate, and monitor health and diabetes education programs to ensure compliance with funding objectives, including outreach, registration, session implementation, curriculum development, wellness activities, data collection, patient education, and reporting.
    Prepare and submit monthly written and oral reports of service activities, and quarterly, semi-annual, and annual reports as required to leadership, IHS, and other agencies as required and within established deadlines.

Supervision Exercised & Professional Development

  • Provides supervision, guidance, and professional development to Diabetes Program staff, ensuring that program objectives and goals are met effectively.
  • Leads, guides, and trains staff, interns, and/or volunteers performing project-related work.
    Participates in recruitment and retention initiatives for staff & volunteers.

Preferred Qualifications:

  • Associates degree in Public Health, Health Administration, Nursing, Nutrition, or a related field preferred. Will consider combination of education and experience.
  • Minimum 5 years of progressively responsible experience in public health, chronic disease prevention, or Tribal community health program management.
  • Demonstrated experience with diabetes prevention and management programs, preferably within Native or Indigenous communities.
  • Minimum of 2 years of experience in leadership role including: staff supervision, program evaluation, and grant management.
  • Strong leadership, coordination, and communication skills.
  • Deep understanding of diabetes prevention, education, and care strategies.
  • Knowledge of public health data collection and evaluation methods.
  • Ability to build and sustain community partnerships and engage diverse populations.
    Proficiency in report writing, program compliance, and funding accountability.
