The Role: What You'll Be Doing (The Impact)
- Become the Wound Whisperer: Lead the charge on all wounds turning complicated cases into clear-cut success stories.
- Next-Gen Healing: Utilize and implement the latest evidence-based practice and tech in wound management—think advanced biologics, and killer documentation skills.
- Patient Advocacy: Be the ultimate patient champion, educating individuals and their families with empathy and clarity on their journey to healing.
- Team Huddle MVP: Collaborate daily with a multidisciplinary squad (MDs, PTs, Nutrition) to strategize and execute holistic care plans. No silo-work here.
- Data-Driven Care: Track outcomes, identify trends, and spearhead Quality Improvement projects to continuously elevate our standard of care.
The Credentials:
- Licensed Nurse Practitioner (NP)
- Wound certification preferred (WOCN, WCC, CWCN, CWS, etc.)
- Experience in SNF/LTC strongly preferred
