Wellness Coordinator Job Summary
Join our caring and dedicated team as a key support to our wellness and care staff! In this vital role, you’ll help ensure residents receive the highest quality of non-clinical care and coordination—from scheduling medical appointments to organizing care plans and supplies. You’ll work closely with the Resident Care Director and other team members to support daily operations, communicate with families, and help new associates get off to a great start. Responsibilities:
- Must maintain strict confidentiality in compliance with HIPAA practices.
- Serve on, participate in and attend various committees of the community.
- Helps coordinate with Sales Director to ensure required nursing forms are returned to the community prior to assessment/move in.
- Partner with the front desk to follow up on the coordination of residents’ medical appointments and transports as needed.
- Ensures daily task sheets are printed and assists with Carestream App.
- Oversees all non-clinical needs of the residents, follow up with families for supplies needs, set up referrals, follow up with outside provider faxes, etc.
- Oversees NBR Continence Program, forwarding all appropriate resident billing to the Business Office Director.
- Oversees and orders all supplies for the wellness department as directed by RCD (Gloves, Med supplies, PPE).
- Schedules Service plan meetings and maintains calendar of meeting schedules.
- Attends Tracking Meetings and schedules change of condition service plan meetings as needed.
- Responsible for ordering of uniforms as directed by RCD.
- Provides care associate assignments for each shift daily in coordination with the Resident Care Director.
- Provides support with orientation and training for new care associates and MNAs/CRMAs
- Assist in the filing of paperwork such as task sheets, communication logs and other paperwork as needed.
- Assist the RCD to ensure that all resident forms are up to date with the most current information.
- Ensure all residents are treated fairly, and with kindness, dignity, and respect.
- Review and understand the resident rights completely.

Qualifications:
- Requires current certification in NH as an MNA or LNA, in ME as a CRMA, CNA or PSS or the willingness to attend an NBR PSS training program.
- Must possess the ability to deal tactfully, and maintains a positive working relationship with personnel, residents, family members, visitors, other medical professionals, vendors, and the general public.
- Must possess the ability and willingness to work harmoniously with professional and non-professional personnel and promote positive work relationships.
- Must be willing to seek out new methods and be willing to incorporate them into existing practices.
- Ability to provide professional customer service at all times, leads by example and addresses any customer/residents/staff concerns in a timely manner.
- Proficient in Microsoft Office Suite, Word, Excel, Outlook and other software programs.
- Is dependable and flexible to work various days, shifts as needed.
