Nurse (RN) – Complex Case Unit (High‑Needs Care Coordination)

About the Role
Staffing Solutions Organization LLC (SSO) is seeking a Nurse (RN) to support the Complex Case Unit (CCU) within the Office of MaineCare Services.
In this high‑impact role, you’ll help MaineCare and non‑MaineCare members navigate complex healthcare, financial, and placement challenges when traditional community‑based solutions are no longer sufficient. You’ll work collaboratively with providers, community partners, and multidisciplinary teams to drive safe, timely, and sustainable outcomes for individuals with high and complex needs.
This position supports Maine residents of all ages, including individuals with chronic, acute, developmental, behavioral health, or psychosocial needs.
What You’ll Do
- Provide guidance, problem‑solving support, and education to members, families, guardians, and care teams
- Facilitate multidisciplinary case conferences for individuals without discharge plans until safe placement or resolution is achieved
- Coordinate complex cases across statewide healthcare and human service systems
- Partner with internal and external stakeholders, including hospitals, providers, and community agencies
- Escalate high‑risk or highly complex situations to middle and senior leadership as appropriate
- Maintain collaborative relationships with key DHHS program areas, including:
- Office of Behavioral Health
- Adult Protective Services
- Children’s Protective Services
- Office of Child and Family Services
- Brain Injury Services
- Developmental Disability Services
- Apply professional judgment while maintaining an objective, member‑focused approach to care coordination
Reporting Structure
- Reports directly to the Complex Case Manager
Minimum Qualifications
- 3 years of progressively responsible professional nursing and/or healthcare service delivery experience
- 2–3 years of clinical experience with pediatric and adult populations or prior case management experience
- Active Registered Nurse (RN) license in the State of Maine (Maine State Board of Nursing)
Preferred Knowledge, Skills & Experience
Strong candidates will bring:
- Experience supporting individuals across the lifespan with complex physical, developmental, behavioral health, and psychosocial needs
- Knowledge of MaineCare, DHHS programs, and statewide community‑based resources
- Ability to build effective working relationships with:
- Members, families, and guardians
- Healthcare providers (hospitals, home health, private duty nursing, PCPs, specialists, therapists, and out‑of‑state providers)
- Community agencies and provider associations
- Strong critical thinking, data gathering, and independent decision‑making skills
- Comfort working autonomously while collaborating within multidisciplinary teams
- Excellent communication, organization, and problem‑solving abilities
