Job Summary


 


The Clinical Case Manager works collaboratively with an interdisciplinary team to support caregivers in meeting their mental health, functional, and family-stability goals. This role provides short-term clinical intervention, care coordination, and crisis support while helping families navigate complex systems and access the resources they need.


 


Minimum Qualifications:


 



  • Master’s Degree in Social Work, Marriage and Family Therapy, Professional Counseling or related field

  • Knowledge of the clinical impact of trauma, attachment, diversity, and poverty 

  • Previous experience in case management, social services, behavioral health, or working with families in high-need communities 

  • Demonstrated ability to provide trauma-informed, culturally responsive support 

  • Strong communication, organization, and time management skills 

  • Ability to work both independently and collaboratively in a fast-paced environment 

  • Demonstrated knowledge of community resources and ability to support families in addressing social determinants of health 




Preferred (but not required):


 



  • Clinical licensure or working toward licensure 

  • Experience in medical social work or behavioral health settings 


 




Essential Responsibilities:


 



  • Collaborate with Family Advocates, Adult Therapists, educators, and interdisciplinary team members to support families holistically 

  • Provide short-term individual clinical case management to caregivers seeking mental health support 

  • Work alongside Family Advocates to take on and manage high-acuity Care Coordination cases, providing direct support to stabilize families and guide them through crisis until they transition back to ongoing support

  • Provide intervention and support during periods of high acuity, addressing functional, behavioral, and mental health needs while navigating multiple systems 

  • Respond to client crises and provide therapeutic support, including assessment and intervention related to suicidal risk, behavioral challenges, and other urgent needs, ensuring appropriate resources and warm handoffs 

  • Facilitate psychoeducational and support groups for caregivers and support parent engagement initiatives 

  • Advocate for families by assisting with access to resources and navigating systems such as healthcare, housing, and education 

  • Maintain timely and accurate documentation in the EHR and other agency systems, including intakes, assessments, progress notes, and treatment plans 

  • Track client progress and outcomes to support program evaluation and continuous improvement 

  • Participate in weekly interdisciplinary team meetings and consultations (child, adult, speech, occupational, and music therapy) to support a collaborative and accountable team culture 

  • Build and maintain partnerships with community organizations to expand accessible services for families 

  • Serve as the point of contact for higher education institutions regarding practicum and internship students, including onboarding and coordination with supervising staff 

  • Maintain appropriate professional boundaries and adhere to ethical standards of practice 

  • Participate in weekly supervision (agency and clinical, as applicable) and maintain licensure requirements 

  • Engage in ongoing professional development opportunities

  • Ability to respond effectively in crisis situations and adapt to changing needs
