Summary: The Housing Coordinator is under the direction of the Assistant Director of HHS/CAA. The HC is responsible for implementing the HUD Housing Grant program that is received annually through Housing and Urban Development (HUD) grant funds. The goal is to assist families temporarily as they stabilize and transition to permanent housing.

- Screen clients and complete all intake forms and applications to determine eligibility for the Transitional Housing Program. Prepare Individualized Service Plan, ISP, for each client. Enter data on each family into appropriate data base.
- Provides comprehensive case management for each client to lead them to self-sufficiency. This can include but not be limited to: job search, job training, counseling and other social services.
- Independently maintains accurate management and administrative records as required by HUD monthly and annually for grant compliance. Submits data to Northlake Homeless Coalition HMIS and prepares all reports for Housing Program.
- Prepares and submits all required reports in a timely fashion.
- Has a comprehensive knowledge of community resources and has relationships with community referral and support agencies. Coordinates and collaborates with staff to maintain up to date community resource directory.
- Effectively manages 6-15 families at any given time.
- Attends weekly or as needed meetings
- Assists Case / Outreach Workers in difficult situations.
- Regular, consistent attendance is required
- Any other duties as assigned
Competencies: Employee must be able to operate basic office equipment, office computer and software, and any other specialty equipment needed for the position. Employee must have strong verbal and written communication skills, organizational skills and customer service skills. Must be able to communicate clearly with staff and the general public and provide clear, concise, and accurate information. Employee must have the ability to develop program strategies for public health and assess and report program outcomes. Employee must be able to collaborate and be highly motivated and an independent self-starter. Employee must have basic knowledge of Microsoft Office program and be able to locate, research, and report information on the Internet. Must have strong sense of respect for confidentiality involving clients. Employee must be able to acquire a knowledge of community resources and actively build relationships with community referral and support agencies.
Work Environment: The incumbent works in a typical office environment with little or no exposure to unpleasant environmental conditions or hazards. Primary functions are limited to an office environment. Occasional duties may require travel or working in non-office environment.
Physical Demands: Light work. Light work involves lifting no more than 20 pounds at a time with frequent lifting or carrying of objects weighing up to 10 pounds. Even though the weight lifted may be very little, a job is in this category when it requires a good deal of walking or standing, or when it involves sitting most of the time with some pushing and pulling of arm or leg controls. To be considered capable of performing a full or wide range of light work, you must have the ability to do substantially all of these activities. If someone can do light work, we determine that he or she can also do sedentary work, unless there are additional limiting factors such as loss of fine dexterity or inability to sit for long periods of time.
Required Education/Qualifications: A high school diploma or equivalent GED and a minimum of two years of relevant experience required, Bachelor’s Degree preferred. Excellent problem solving skills; strong, detailed record keeping; multi-tasking and willingness to accept new assignments; excellent customer service, community involvement and outreach are essential. Must possess a valid Driver’s License.
