Job Summary
Job description/Responsibilities: Full-Time: 40 hours AM Shift PAY RANGE: $55.68 - $92.80 JOB SUMMARY: The Case Manager reports directly to the Director of Nursing Services in the SNF and is responsible for coordinating Care Management duties of patients in the SNF, follow up appointment support, insurance verification and works closely with the Interdisciplinary team. The Case Manager also supports discharge planning and works closely with insurance representatives and facility partners. DUTIES AND RESPONSIBILITIES 1. Develops individualized care plans, manage utilization review, insurance authorizations, and facilitates safe discharge planning to home or other levels of care. 2. Keeps the resident care plan and assessment up to date. 3. Evaluates patient needs, goals and rehabilitation potential to create comprehensive care plans. 4. Assumes the overall responsibility for the coordination of patient care planning, case management and actively leads the process. Leads care conferences with physician, nursing staff, therapists and families. 5. Analyzes and monitors facility quality indicators derived from CMS database and notifies the Director and develop action plans. 6. Coordinates services, discharge planning, durable medical equipment, transportation. 7. Coordinates Utilization Review activities, updates payers on patient progress. Works with Business Office to obtain authorizations, track and bill patient days. Assists with collections as needed. Maintains accurate records in compliance with state and federal regulations. 8. Avoids and prevents injury to self and patients by applying the principles of body mechanics. Maintains a safe environment, functioning with an awareness and application of safety issues identified within the unit/facility, i.e., suction and oxygen immediately available when needed. 9. Recognizes signs of abuse and reports timely and appropriately to the Unit Abuse Coordinator. 10. Performs other duties as assigned or required.

Required Qualification: EDUCATION, EXPERIENCE, TRAINING 1. Current and valid RN license issued by the State of California required. 2. Current Basic Life Support (BLS) required. 3. Computer skills required. 4. Minimum of two years recent SNF experienced preferred. 5. Sexual Harassment/ Bullying Prevention Certification required.
