- The Community Hospice has a rewarding opportunity for an RN Supervisor to lead our RN Case Managers, LPNs and Home Health Aides who are caring for Hospice patients/families at Capital Region nursing homes.
- Coordinate closely with the team's RN Clinical Lead, other RN Supervisors, the hospice interdisciplinary team, and Directors to ensure high quality end of life care to patients and families.
- This is an ideal opportunity for someone who enjoys collaborating closely with a team (our teams include physicians, RNs, LPNs, aides, social workers, chaplains, complementary therapists, and volunteers!).
- A current license to practice as a Registered Nurse in the State of New York
- Associates degree in Nursing required. BSN preferred.
- Prior Supervisory experience strongly preferred.
- Minimum of 3 years Hospice, acute care, or homecare RN experience required.
- Prior Hospice experience strongly preferred
