Hospice RN Case Manager is part of a collaborative, interdisciplinary team who provides high-quality, person-centered end-of-life services/support including those living at our Kaukauna and Green Bay campuses as well for those living in a 5-county territory. RN Case Manager works collaboratively with patients, clinical teams, and family members to ensure physical comfort and symptom management based on individualized assessments and patient-specific needs and goals.
We have a part-time position that requires 64 hours every 2 weeks (primarily Days), with an on-call rotation every 4th week. After hours, weekend hours, and holidays required only per client critical needs. This is a benefits eligible position.
Requirements: RN licensure, prefer Hospice experience but not necessary.
