Schedule:Â Full Time Monday-Friday 8AM-5PM

$5,000 Sign-On Bonus!

50% at 6 months of employment

As a RN Case

Manager

You will:Â

• Assess/
  
  

monitor

physical, emotional, and psychological needs of patients

• Create hospice care plans that align with the
  
  

patient's

wishes and goals

• Direct nursing care: administering medications, treatments, and interventions
• Provide pain and symptom managementÂ
• Educate and support the patient's family and caregivers
• Collaborate with interdisciplinary team
• Maintain
  
  

accurate

and

timely

documentationÂ

• Participate in on-call rotation as required by local branch

Qualifications

Required

• Graduate of an accredited nursing school with current licensure to practice in state of operation
• One year nursing experience
  

, 3 years preferred

• Experience in hospice or a similar setting preferred
• Knowledge of the hospice philosophy of care
• Commitment to clinical and documentation excellence
• Must be a licensed driver with an automobile that is insured
  
  

in accordance with

state/or organization requirements and is in good working order
