Provides Physical Therapy services to residents by assisting in the treatment, program planning and implementation, related documentation, and communication and functions.

Job Summary:

We are seeking a Physical Therapist to join our dedicated team at St. Otto's Care Center. In this role, you will play a vital part in helping our patients regain their independence and improve their quality of life through tailored therapeutic interventions.

This will be a Part Time Position, 48 hours a pay period, Monday through Friday. Weekend and Holiday coverage as needed (can discuss in person).

Qualifications:

Bachelor of Science or Arts degree in Physical Therapy from an accredited program.

Successful completion of National Certification Examination for Registered Physical Therapist.

Current state licensure.

Essential Processes:

1. Documents result of resident’s assessment, treatment, follow-up and termination of services. Adheres to Physical Therapy Department procedures regarding documentation of Physical Therapy services.

2. Accurately and efficiently utilizes documentation and billing software to record patient treatment, labor and CPT codes according to St. Otto’s Care Center procedures.

3. Maintains safe and clean work area and adheres to facility/company safety standards. Complies with all Infection Control, Universal Precautions, and OSHA standards for the healthcare professional.

4. Reports to work on time and coordinates schedule to achieve maximum productivity and efficiency during assigned shift. Maintains regular attendance to ensure patients get proper therapy.

5. Assists with and participates in Quality Assurance Program as required.

Duties:

1. Provides Physical Therapy services to residents (the term “resident” applies to patients, guests, clients) in a safe and effective manner.

2. Conduct screenings of residents newly admitted to the facility and referred by nursing to determine need for intervention/treatment.

3. Evaluates residents to obtain data necessary for treatment planning and implementation. Conducts specialized evaluations as indicated.

4. Interprets and communicates evaluation findings to residents, family and other team members.

5. Establishes individual Resident schedules in conjunction with other team members.

6. Develops treatment plans by identifying a problem list, including short and long term goals and methods to achieve identified goals.

7. Coordinate treatment plan with resident, family and other team members.

8. Implements and conducts treatment as outlined in treatment plan.

9. Monitors resident’s response to intervention and modifies treatment as indicated to attain goals in accordance with physician orders.

10. Develops appropriate home or community programming to maintain and enhance the performance of the resident in their own environment.

11. Documents results of resident’s assessment, treatment, follow-up and termination of services.

12. Adapts treatment to meet the individual needs of the resident.

13. Provides/recommends/fabricates adaptive devices, orthotics or prosthetics. Trains resident, family, and appropriate staff in the use or application of the above.

14. Initiates referrals when appropriate and in accordance with St. Otto’s Care Center procedures.

15. Participates in discharge planning.

16. Supervises and directs PTA's and COTA’s in the implementation of the treatment plan.

17. Supervises students and volunteers.

18. Supervises Rehabilitation Aides.

19. Reviews the quality and appropriateness of the total services delivered and of individual occupational therapy programs for effectiveness and efficiency, using predetermined criteria.

20. Participates in resident, family and staff education.

21. Attends in-service meetings and facility meetings as required.

22. Provides in-service education to the members of the residents care team.

23. Assumes responsibility for ongoing continuing education and professional development.

24. Provides treatment in accordance with established standards of practice, department procedures and productivity standards.

25. Consistently demonstrates sound judgement in the evaluation, planning, implementation, and follow-up of resident therapy programs.

26. Assists with and participates in Quality Assurance Program as required.

27. Projects a positive and professional image at all times.

28. Attends in-service meetings and facility meetings as required.

29. Assumes responsibility for ongoing continuing education and professional development.

30. Performs other duties as assigned.

Pay: From $40.00 per hour
