What are the responsibilities and job description for the Social Worker Long Term Care position at St Marys Healthcare?
Maintaining and/or improving resident’s ability to control everyday physical needs and mental/psychosocial needs
Initiating appropriate interventions for residents experiencing mental or behavioral health issues
Identifying individual social and emotional needs
Participate in the planning of the resident’s admission, community re-entry, or transfer to another facility
Work with the finance team to ensure the resident is educated and the proper documents are in place for the resident during his or her stay
Work with the resident on discharge and get necessary DME or home care in place so the resident is safe when he or she returns to the community
Other duties as required for the safety and care of the residents
Qualifications:
Nursing Home or Hospital experience. Internship is sufficient. (Required)
BSW, MSW or BA/BS in related field. (Required)
Bi-lingual English/Spanish (preferred but not required)
Education:
- Bachelor's Degree required
- Minimum years of experience - 1 year

Salary : $25 - $32
