In 1997 our Adult Day Health Care (ADHC) opened. St. Mary's Center expanded to provide medical care and social support to people in the community living with HIV/AIDS. Today the program offers more than 60 types of support groups and services each week. We recognize that fun is an important part of health and we offer numerous recreational opportunities to make fun a part of wellness.
St. Mary’s Center is hiring for a full-time Program Director, Adult Day Health Care Program (ADHC). The Program Director is responsible for the day-to-day operations of the Adult Day Health Care Program to include social work, substance abuse, mental health, clinical services, and outreach.
RESPONSIBILITIES:
- Responsible for the administrative and overall operation of ADHC Program.
- Supervise all social workers and substances abuse staff & monitors chart documentation.
- Oversee all aspects of psychosocial programming and services.
- Coordinate the development, implementation and evaluation of case management regimen for clients through assessment, care planning, and monitoring care.
- Ensure admission and discharge procedures are consistent with client need.
- Liaison with outside regulatory and governmental agencies as needed.
- Provide counseling to clients in the Adult Day Health Care Program and Amida Care (SNP).
- Leadership role during the annual NYSDOH-AIDS Institute and IPRO survey process; prepare plan of correction, as needed.
- Provide quarterly audits on all ADHC/Amida Care (SNP) charts.
- Conduct weekly groups for clients and completes the necessary documentation.
- Chair QAPI (Quality Assurance and Quality Improvement Activities for quarterly reports.
- Member of the Comprehensive Care Plan (CCP) team and attends weekly meetings.
- Provide case management for a select caseload.
- Revise & update departmental manuals as needed.
- Oversee outreach and marketing to community organizations to acquire referrals.
QUALIFICATIONS:
- Masters Degree in Social Work
- 2 years experience in working in a healthcare setting, people living with AIDS, mental health, and or substance abuse
- Knowledge of NYSDOH survey process
- 2 years supervisory experience
- Active NYS LMSW or LCSW preferred
- Bilingual (Spanish) preferred
SALARY: $80,000 - $85,000 annual
