What are the responsibilities and job description for the MDS Nurse position at St. Luke Hospital and Living Center?
The MDS Coordinator is resopnsible for completion, timeliness and accuracy fo Minimum Data Sets for residents in the long-term care facility as mandated by law. This person identifies problem areas indicated by the MDS, then assists and follows up with appropriate staff members to develop and implement modified care plans.
Essential Functions
- Asses, plan and prioritize each assigned resident's nursing needs according to current standards of nursing practice and CMS guidelines.
- Completes and consistently documents all information on the MDS within the appropriate timeframe. Collaborates with other sources - providers, nurses, aides, therapists, resident chart and family members to obtain appropriate data.
- The MDS process leads to the creation of an individualized plan of care and development of quality measures.
- The care plan created outlines the plan of care developed by the team through the MDS and other assessments established according ot state and federal regulations.
- Care planning considers the resident's medical, physical, cognitive, economic and emotional strengths and abilities as well as their available support system.
- The resident's care plan/problem list is evaluated and reprioritized as the needs of the resident change.
- Management and organization of resident medical records.
- Receive and process orders.
- Chart audits and review.
- Maintain confidentiality of medical records information at all times.
- Strong skills with various computer systems including Microsoft Office, Outlook and EHRs is a must.
This position requires a current Kansas nursing license. At times the MDS Coordinator will be expected to assist with patient care, but functions primarily as an administrative nurse. The MDS Coordinator must be able to assist with movment of residents, respond rapidly to emergencies, lift and carry up to 35 pounds, and consistently sit, stand, squat, reach, walk, bend, twist, and kneel.
