Licensed Practical Nurse (LPN) Adult Day Services - Part Time

This part-time position (20 hours per week) offers a meaningful opportunity to provide compassionate care and support to individuals in a community-based setting.

What You’ll Do

• Provide direct nursing care and health monitoring for individuals in the Adult Day Service program.
• Administer medications and treatments as prescribed.
• Collaborate with program staff to support individuals’ health and wellness goals.
• Maintain accurate medical records and documentation.
• Communicate with families, caregivers, and healthcare providers as needed.
• Participate in team meetings and contribute to individualized service planning.
• Ensure compliance with regulatory standards and organizational policies.
• Assist individuals with personal care needs including hygiene, toileting, and feeding.
• Support individuals in community outings and activities.
• Monitor and respond to health emergencies and implement appropriate interventions.

What’s Required of You

• Current LPN license in the state of Pennsylvania.
• Experience working with individuals with intellectual and developmental disabilities preferred.
• Strong communication and interpersonal skills.
• Ability to work independently and as part of a team.
• Commitment to person-centered care and the mission of Saint Joseph’s Center.
• Ability to lift and transfer individuals as needed.
• Basic computer skills for documentation and communication.
