As a Physical Therapist Per Diem, you will play a vital role in our multidisciplinary team, providing exceptional patient care and rehabilitation services to our residents. Responsibilities:

• Provide physical therapy services to residents, including assessment, treatment planning, and implementation of plans

• Develop and implement individualized treatment plans tailored to each resident's needs

• Collaborate with other healthcare professionals to ensure effective communication and coordination of care

• Conduct evaluations and assess residents' progress, making necessary adjustments to treatment plans

• Maintain accurate and detailed records of patient care and treatment

• Participate in staff meetings and educational in-services to stay current with industry best practices

Requirements:

• Graduate degree in Physical Therapy from an accredited program

• Current licensure to practice as a Physical Therapist in the state of Vermont

• Strong communication and interpersonal skills

• Ability to work effectively as part of a team and adapt to changing priorities

• Strong organizational and time management skills
