We are seeking a highly skilled Licensed Practical Nurse (LPN) to work on our evening shift, providing exceptional patient care and contributing to our facilities' mission to set the standard of care.

Responsibilities:

• Provide direct patient care to residents, including administering medications, wound care, and vital signs monitoring

• Develop and implement individualized care plans, in collaboration with the healthcare team

• Maintain accurate and detailed records of patient assessments, treatments, and progress

• Collaborate with other healthcare professionals to ensure high-quality patient care and outcomes

• Participate in quality improvement initiatives and contribute to creating a positive and supportive work environment
