What are the responsibilities and job description for the Case Manager position at St. Francis House?
$1000 HIRING BONUS! Due to program expansion, the St. Francis House is seeking to hire a full time Case Manager to provide direct services to the individuals in our program. Duties include, but are not limited to doing initial assessments of individuals program eligibility, developing short term and long term goals as well as being the main contact person for individuals on their case loads. In addition, case manager will meet with individuals to assist them with their needs: education/employment/life skills/goal planning and ongoing assessments in relation to chemical dependency. Work closely with community resources to provide referrals as needed.
Bachelor's degree or related work experience in social services. Minimum of two years providing direct support to individuals, knowledge of community resources and ability to communicate well with others. Strong written and organizational skills. Hours will vary depending on the individuals on your caseload, but flexibility will be required depending on the needs of the individuals being served.
