Registered Nurse – Home Health (Full Time/Part Time)

What You’ll Do
- Complete OASIS assessments (Start of Care, Recert, Resumption, Discharge)
- Provide skilled nursing services: wound care (including complex wounds), IV therapy, and medication management
- Support chronic disease management and patient/family education
- Work closely with PT/OT/ST and the full interdisciplinary team
- Follow CMS, Medicare, and PA DOH standards with confidence and accuracy
- Hourly pay model (not per-visit) — more time for patients, documentation, and care
- Flexible scheduling with supportive clinical leadership
- Streamlined EMR and strong office support
- Growing census

- Current RN license in Pennsylvania
- Prior home health experience preferred
- Strong knowledge of OASIS and CMS Guideline

Qualifications of the position:
- Current RN licensure in Pennsylvania
- Valid driver’s license, auto insurance, and reliable transportation
- Current CPR certificati
