1. To the Program Supervisor
2. For performing 32 hours of work weekly
3. For following all policies and procedures of St. Augustine Youth Services
4. For participating in all supervision, staff meetings, and treatment team meetings.
5. For attending all required meetings including but not limited to: Multidisciplinary Team (MDT) meetings, IPT (Integrated Practice Team), school and other community providers.
6. For providing the five functions of case management: planning, assessment, linking and advocacy, monitoring and discharge planning.
7. For acting as case manager for up to 1-3 children.
8. For conducting the initial assessment and developing the recipients service plan according to Florida Medicaid guidelines.
9. For working with the recipient and the recipient's family to address issues related to the implementation of the service plan.
10. For assessing the effectiveness of the service plan in meeting the identified needs of the recipient.
11. For linking and facilitating the recipient with appropriate services and resources identified in the service plan through referrals to reach desired goals.
12. For advocating for the acquisition of services and resources necessary to implement the service plan by representing or defending recipients through direct intervention.
13. For participating in the recipient's individualized treatment plan development or individualized services plan review.
14. For coordinating the delivery of services as specified in the service plan with the help of the recipient, the recipient's family, and the recipient's natural support system. This may include providing transportation as needed.
15. For monitoring service delivery to evaluate the recipient's progress.
16. For documenting mental health targeted case management activities in accordance with Florida Medicaid documentation requirements.
17. For completing all required documentation and assisting with data and program reporting as needed.
18. For maintaining client records including client charts and electronic health records.
19. For crisis intervention/support by assisting recipients in crisis in getting access to the necessary resources in order to cope with the situation.
20. For arranging and coordinating discharge and after care/follow-up services as needed.
21. For conducting clinical care Medicaid recipient staffing as needed, in which the case manager is meeting, with either the recipient's treatment team or one-on-one with one of the following individuals: psychiatrist, psychiatric ARNP, physician, therapist, teacher, attorney, guardian ad litem, or any other professional who is directly serving the recipient.
22. For completing service plans; plan updates, progress reports and other required reports in the manner described by the policies and procedures.
23. For maintaining a list of community providers for referral services.
24. For performing other duties as required.

Qualifications

•   Have a bachelor's degree from an accredited university or college with a major in counseling, social work, psychology, criminal justice, nursing,  rehabilitation, special education, health education, or a related human services field (a related human services field is one in which major course  work includes the study of human behavior and development) and have a minimum of one year of full time or equivalent experience working with  children with serious emotional disturbances; or Have a bachelor's degree from an accredited university or college and three years full time       or equivalent experience working with children with serious emotional disturbances.
•   Have knowledge of available resources in the service area for children with serious emotional disturbances.
•   Is knowledgeable of and comply with state and federal statutes, rules and policies that affect the target population.
•   The ability to work cooperatively as a member of a team for the benefit of the children and the organization.
•   The ability to interface in a professional manner with the community
•   The ability to communicate effectively, to other members of the treatment team, difficulties that children may be experience                       
•   The ability to facilitate community outreach programs.
