The Role
Lead all care-management functions across the acute-care ministry — case management, utilization management, discharge planning, and social work. Accountable for patient outcomes, throughput, and cost, this role sets strategy and drives performance against regional/national benchmarks.
What You'll Do
- Lead case management, utilization review, discharge planning, and social work to ensure safe, timely transitions of care
- Drive throughput and quality metrics through data-based performance improvement
- Recruit, develop, and lead a high-performing care management team
- Partner with physicians, executives, and community providers on utilization and discharge outcomes
- Own department budget, staffing, and financial performance
- Ensure regulatory compliance and report to senior leadership/governance
Requirements
- BSN and Master's degree in a health-related/science field
- 2 years acute-care case management experience
- 2 years leading case management teams
Licenses & Certifications
- Active California RN License (required at hire)
- California Fire and Life Safety Card (within 30 days of hire)
Pay: $250,000.00 - $325,000.00 per year
