Assignment Duration: 13 Weeks Contract
Schedule: Monday–Friday, 8:00 AM – 5:00 PM
Hours: 40 hours per week
Weekly Gross: $2700-2800/week
Job Summary
We are seeking a dedicated and experienced Registered Nurse (RN) Case Manager to join our team in Syracuse, NY. The RN Case Manager will be responsible for coordinating patient care, ensuring appropriate utilization of services, and supporting patients through the continuum of care.
Requirements
- Active Registered Nurse (RN) license in the state of New York
- Minimum of 1–2 years of case management or relevant clinical experience
- Strong knowledge of care coordination and discharge planning
- Excellent communication and organizational skills
- Ability to work independently and collaboratively within a team
