Multiple Part-Time Positions looking for weekend availability
Overall Function:
Maintains the general safety and security of residents at Galleton Commons and aids residents with the implementation of their individualized housing support service plans.
Essential Functions:
- As a member of the housing support team, provides hands-on support and assistance to Galleton Commons residents as identified in their individualized housing support service plans.
- Provides hands-on instruction to residents to implement crisis plans and assist with crisis resolution.
- Conducts recreational and social activities with the residents.
- May assist in meal preparation for the Galleton Commons residents.
- Provides transportation to medical appointments as necessary.
- Participates in regular staff meetings and trainings to develop basic knowledge and skills to assist residents in crisis resolution and to support proper job performance.
- Provides crisis support as needed to the residents at Galleton Commons.
- Maintains overall cleanliness of Galleton Commons grounds and reports any repair needs to the supervisor.
- Reports to the supervisor any resident needs that would affect their current individualized housing support service plan.
- Maintains proper documentation of all assistance and support provided to each resident as identified by their individual housing support service plan.
- Other duties as required.
- Adheres to all policies, laws, regulations, and codes of ethics and confidentiality as outlined by federal and state laws, and agency policies and procedures.
Qualifications
- High School Diploma, GED, or an equivalent combination of skills, experience, and education may be considered at the discretion of the Agency.
- Ability to receive, understand, and communicate information with effectiveness, and follow instructions,
- Must have a valid PA Driver’s License and a safe driving record.
- Must have the ability to utilize the electronic medical record.
- Must successfully obtain Criminal History Clearance from the PA State Police, Child Abuse Clearance from the PA Department of Human Services, and FBI Fingerprint Clearance.
- Must not be excluded from participation in Medicare, Medicaid, or any other federal health care program.
- Ability to transport oneself and/or individuals in an insured vehicle, as program needs dictate.
- Ability to safely operate a variety of agency vehicles.
- Ability to assist, move, and/or transfer residents weighing up to 150 pounds may be required.
- Ability to assist, move, and/or transfer program equipment and supplies weighing up to 30 pounds.
