Responsibilities:- Coordinate and manage comprehensive hospice care plans tailored to each patient's unique needs.
- Conduct regular home visits to assess patient conditions and adjust care plans as necessary.
- Collaborate with interdisciplinary teams to ensure seamless communication and continuity of care.
- Educate patients and families on hospice services, ensuring they understand and feel supported throughout the process.
- Document patient progress and care activities accurately and timely, maintaining compliance with regulatory standards.
- Provide emotional support and guidance to patients and their families, fostering a compassionate care environment.
- Participate in team meetings and contribute insights to enhance patient care strategies and outcomes.
- Experience in hospice or palliative care, ensuring patients receive compassionate and tailored support.
- Ability to coordinate and manage comprehensive care plans, adapting to each patient's evolving needs.
- Proven track record of effective collaboration with interdisciplinary teams to enhance patient care.
