Compensation:

$53 - $56 hourly

Responsibilities:
  • Coordinate and manage comprehensive hospice care plans tailored to each patient's unique needs.
  • Conduct regular home visits to assess patient conditions and adjust care plans as necessary.
  • Collaborate with interdisciplinary teams to ensure seamless communication and continuity of care.
  • Educate patients and families on hospice services, ensuring they understand and feel supported throughout the process.
  • Document patient progress and care activities accurately and timely, maintaining compliance with regulatory standards.
  • Provide emotional support and guidance to patients and their families, fostering a compassionate care environment.
  • Participate in team meetings and contribute insights to enhance patient care strategies and outcomes.
Qualifications:
  • Experience in hospice or palliative care, ensuring patients receive compassionate and tailored support.
  • Ability to coordinate and manage comprehensive care plans, adapting to each patient's evolving needs.
  • Proven track record of effective collaboration with interdisciplinary teams to enhance patient care.
  • Strong communication skills to educate and support patients and families, fostering understanding and trust.
  • Proficiency in documenting patient progress and care activities, maintaining compliance with regulatory standards.
  • Ability to provide emotional support and guidance, creating a compassionate care environment.
  • Experience in conducting home visits, assessing patient conditions, and adjusting care plans as needed.
