Case Manager- Outpatient Services
This position is based at our Henry County Clinic in Abbeville, Alabama.
Work hours of this position are Monday-Friday 8am-5pm
Duties of this position include:
- Providing initial case management services
- Implementing, updating and revising service plans as needed
- providing family support and education
- Basic living skills
- Identifying appropriate community resources as needed
Requirements:
- BS Degree in a mental health related field
- Good MVR, willing to submit to motor vehicle report check
- Must be willing to submit to a pre-employment background check and drug screen
- Must successfully complete Case Management training and receive certification from the Department of Mental Health
- Prefer previous experience providing patient care in a mental health or substance abuse treatment environment
- Must meet insurability requirements to drive a system vehicle (21 years of age).
