What are the responsibilities and job description for the Patient Navigator position at SPECIAL HEALTH RESOURCES FOR TEXAS INCORPORATED?
The CCBHC Navigator/Case Manager will assess patients and work with behavioral health providers and counselors to develop a treatment plan, choose the necessary resources for each patients’ mental health and substance use disorder needs, set up referrals to BH providers and LCDCs. The patient navigator/case managers will coordinate services and connect clients with resources that can help them with their challenges, whether they be psychiatric illness, substance abuse or other personal problems that impact their ability to function normally in everyday life.
- Provide education and enrollment services to targeted population
- Assessing patient needs initial, crisis, and linkage to care (behavioral health or substance use disorder).
- Coordinating and monitoring patients’ services, referral to behavioral health providers and substance use counselors.
- Complete required reports and supportive information to Project Coordinator in a timely manner
- Track progress toward goals and regularly communicate with Project Coordinator, Behavioral Health Provider and LCDC about patient as needed.
- Work collaboratively with Project Coordinator and Director of Quality by assisting the members of the advisory work group (AWG).
- Assist with CCBHC surveys and needs assessments.
- Attend on-going education, meetings and training required by the CCBHC project.
- Schedule, coordinate, and deliver community educational presentations, seminars, and outreach events as well as provide enrollment assistance to target population
- Provide excellent customer service to both internal and external clients
- Requires valid driver’s license, appropriate driving record, and auto liability insurance
- High School or GED required
- Ability to multi-task
- Computer literacy required
- Excellent people skills to establish rapport and put others at ease
- Experience as a behavioral health patient navigator or case manager preferred
- Experience working with federal grants, project evaluator and government project officers
- Adequate knowledge about behavioral health and substance use disorders
- Proficiency in Word, PowerPoint, and Excel
- Ability to learn funding sources and organization software and/or electronic health systems.
- Capacity to work independently and interdependently
- Excellent and effective oral and written communication skills, with strong group presentation skills
- Strong analytical, and writing skills
