What are the responsibilities and job description for the CCU CASE MANAGER position at Southwestern Illinois Visiting Nurse Association?
Case managers conduct in-home assessments for older at-risk adults and work with them to put together a care plan that enables them to safely maintain their independence. This full-time position includes generous paid time off, benefits and mileage reimbursement. May work from home at least one day per week. Hours are 8 am to 4 pm Monday through Friday. No social work license required but must have a BA/BS in social sciences, social work, or human services related field.
Job duties include:
- Administer determination of need including a comprehensive needs assessment
- Develop an individualized and person-centered plan of care
- Make appropriate referrals and respond to client requests
- Authorize services as needed
- Maintain case records and provide timely documentation
Salary : $23 - $26
