Southwestern Behavioral Healthcare is looking for a full-time State Psychiatric Hospital Liaison to join our team and help coordinate quality care for adults living with serious and persistent mental illness. In this role, you’ll serve as a key connection between State Psychiatric Hospital (SPH) settings and Community Treatment Facilities, coordinating patient transfers, sharing important information, and helping ensure continuity of care throughout the transition process. WHAT IS THIS POSITION RESPONSIBLE FOR?
- Provide school and community-based case management, skills training/development, and other services to children and their families.
- Work with children and their families referred from the school as well as the Department of Child Services, Juvenile Probation, System of Care, Southwestern assessment team and therapists, and other referral sources.
- Maintain appropriate records of services to reflect patient progress and document treatment plans and goals.
- Maintain documentation and submit reports according to established time frames.
- Communicate professionally, make referrals and coordinate services with community agencies.
WHAT'S REQUIRED FOR THIS POSITION?
- Must hold a Bachelor's degree in a mental health or humans services related field with a minimum of two years of experience working with long-term psychiatric patients.
- Must be at least 21 years of age, maintain a valid Indiana driver's license and valid car insurance.
- Must obtain and maintain crisis management and verbal de-escalation training.
