The Licensed Clinical Social Worker/Licensed Professional Counselor/Licensed Marriage and Family Therapist (LCSW/LPC/LMFT) is responsible for providing high quality patient care, displaying good skills in assessment, diagnosis, and planning for the care of patients in the Southwest Medical Group. The LCSW/LPC/LMFT is expected to function as a member of the multidisciplinary team and participate in the medical group activities. The LCSW/LPC will be required to visit patients at multiple clinic locations within the Southwest Medical Group and will interface with the discharge planning and social work staff of Southwest Memorial Hospital.
Regulatory Requirements:
- Licensed Clinical Social Worker/Licensed Professional Counselor/Licensed Marriage and Family Therapist or license eligible in state of Colorado.
- Master's Degree in social work, counseling or psychology from an accredited university.
- Current BLS certification.
- One (1) year experience working with adult, pediatric or geriatric patients.
Language Skills:
- Able to communicate effectively in English, both verbally and in

- Basic computer knowledge.
