JOB SUMMARY
Reporting to the Director of Nursing, the Licensed Practical Nurse (LPN) ensures the general well-being of residents by providing nursing care and services for residents in a manner and environment that provides dignity and full recognition of each resident’s individuality. The LPN assists the RN with responsibility for total resident care within the unit. The LPN respectfully interacts with all residents, family members, visitors, colleagues and staff. This is a safety sensitive position.
ESSENTIAL FUNCTIONS

- Provides total nursing care for skilled nursing facility residents
- Assures resident safety
- Observes and reports symptoms and conditions of residents
- Maintains timely and accurate records on residents’ condition, medication and treatment
- Assists with developing the resident care plans; evaluates and revises plans as needed
- Completes monthly summaries on each patient
- Dispenses and charts all medications
- Admits and orients new residents and families
- Assists in scheduling appointments and arrangements for transportation for medical services in consultation with RN
- Assists in maintaining a physical, social and psychological environment that is conducive to the best interest and welfare of the patients
- Assists physicians as needed and requested
- Requisitions supplies and services
- Assists in the training and supervision of nursing service personnel
- Regular and reliable attendance is an essential function of this job

WORKING CONDITIONS
Work is conducted primarily in health care facility. There is possible exposure to communicable diseases and blood and body fluids that may contain HIV and/or HBV. This position frequently requires working within strict deadlines, under stressful conditions and with a variety of personalities, who may at times be emotional.

Qualifications:MINIMUM QUALIFICATIONS
- Associate’s degree in nursing from an accredited college or university
- Licensed Florida LPN; required to maintain licensure
- Completed LPN Supervisory Course or ability to complete within 6 months of employment
- Current CPR certification required
- Two or more years of related experience and/or training in geriatrics in long term care, home health or other health care setting
- Excellent verbal and written communication skills
- Computer literacy which includes, but is not limited to, Microsoft Office suite
