Job Summary
The VA Community Homecare Coordinator/Liaison serves as the primary point of contact between the VA, veterans, and our home care agency. This role ensures seamless coordination of care, timely communication, and compliance with VA regulations. Responsibilities
- Act as the main liaison between the VA, veterans, families, and internal care teams.
- Coordinate and manage veteran referrals and authorizations for home care services.
- Ensure all VA documentation, billing, and reporting requirements are met accurately and on time.
- Conduct outreach and relationship-building with VA case managers, social workers, and community partners.
- Assist in scheduling and coordinating care plans based on veterans' needs and VA service authorizations.
- Monitor client satisfaction and follow up on service-related concerns promptly.
- Maintain confidentiality and compliance with HIPAA and VA guidelines.
Qualifications:
Required
- Experience in healthcare coordination, case management, or a related field.
- Strong knowledge of VA community care programs and authorization processes.
- Excellent communication and interpersonal skills.
- Proficient in documentation, electronic health records (EHR), and Microsoft Office Suite.
- Ability to multitask and manage competing priorities in a fast-paced environment.
Preferred
- Background in home health care, hospice, or skilled nursing services.
- Prior experience working with the VA or veteran populations.
- Associate's or Bachelor's degree in healthcare, social work, or a related field.
