Job Description Summary:

The Licensed Practical Nurse [LPN] supports the Center Director and Registered Nurse in delivering high-quality health care services and daily care to program participants in accordance with COMAR 10.27.10. The LPN is responsible for health monitoring, medication administration, documentation, and assisting participants in accordance with individualized plans or care.


Key responsibilities include recording participant observations, conducting monthly health checks, leading daily exercise programs, supporting Activities of Daily Living (ADLs), assisting with meals, maintaining a safe and clean environment, and supporting administrative and reporting duties. The LPN also assists with medical supply inventory, participant outings, emergency response, and other direct care needs.


The LPN reports directly to the Center Director and is accountable for the effective, professional performance of all assigned duties. The ideal candidate will consistently demonstrate compassion, professionalism, and adherence to nursing standards and agency policies.


Position Requirements Summary:

Education & Experience:

  • Required: High School Diploma or equivalent. 
    • Current Maryland LPN license is in good standing.
  • Experience: Minimum of three years’ experience in a health care setting.

 

Technical Skills:

  • Strong team-building and collaboration skills; able to maintain positive working relationships.
  • Professionalism, flexibility, discretion, and cultural sensitivity in all interactions.
  • Proficiency in Microsoft Word, Excel, Outlook, and related applications for data entry, reporting, and documentation.
  • Ability to always maintain confidentiality in accordance with laws, regulations, and agency policy.

 

Communication Skills:

  • Strong verbal and written communication abilities.
  • Ability to interact professionally with participants, families, colleagues, and multidisciplinary team members using tact and diplomacy.
  • Ability to communicate clearly and appropriately with coworkers, supervisors, customers, and vendors.


Responsibilities and Duties:

Care Delivery & Health Monitoring:

  • Follow each participant’s individualized plan of care and provide input to the multidisciplinary team.
  • Conduct and document monthly health monitoring, including weight and vital signs.
  • Record participant observations, functional levels, absences, and occurrences in medical records.
  • Report significant findings or changes in physical or mental condition to the Registered Nurse.
  • Assist participants with Activities of Daily Living (ADLs) in accordance with COMAR 10.27.10.
  • Lead the daily physical exercise program, providing safe routines tailored to seniors and individuals with limited mobility.
  • Assist with preparing, serving, and supporting participants during meals, including cleanup.

 

Medication & Treatment Administration:

  • Administer medications and treatments, including insulin, nebulizer treatments, blood glucose monitoring, and GT feedings.
  • Renew physician orders; coordinate medical appointments; request prescription refills from pharmacies.

 

Safety, Emergency Response & Environment:

  • Initiate CPR in the event of cardiac or respiratory arrest.
  • Initiate emergency assistance in accordance with agency policies and procedures.
  • Maintain participant confidentiality and adhere to nursing practice standards.
  • Assist with maintaining a clean, sanitary, and safe environment.

 

Administrative & Program Support:

  • Assist the Center Director with data collection, analysis, and monitoring of monthly reports.
  • Complete monthly attendance reports for Medicaid reimbursement.
  • Maintain inventory of medical supplies; assist with various administrative tasks as assigned.
  • Accompany participants on field trips and assist with agency vehicle transportation as needed.
  • Participate in staff development programs relevant to Home Health Care and current nursing practices.
  • Support participant groups and engage in individual activities as needed.
  • Assist the Center Director with recruitment activities.

 

Compliance & Professionalism:

The LPN must demonstrate knowledge of and compliance with:

  1. COMAR 10.27.10
  2. Applicable nursing practice standards
  3. Relevant statutes and regulations governing LPN practice
  4. Agency policies and procedures


Perform other duties as assigned.


Licenses or Certificates:

  • Current Maryland LPN license is in good standing.
  • CPR and First Aid certification required.


Special Requirements:

  • State and Federal Bureau of Investigation Criminal Background Check.
  • National Sex Offender Public Registry Check.
  • Physical examination and Tuberculin PPD Tine Test at time of employment and annually (or as required by physician).
  • Pre-employment, random, and post-accident drug and alcohol testing.

    • Lifting as patient needs require
