Southern Health Partners is seeking a compassionate and dedicated Licensed Practical Nurse (LPN) to provide chronic care services at the Madison County Jail in Alabama.

What You'll Do

What You'll Do
As a Chronic Care LPN, you'll play an important role in helping patients manage ongoing health conditions and receive consistent, quality care.
- Provide nursing care to patients with chronic and ongoing healthcare needs
- Monitor patients' conditions and identify changes that may require additional evaluation
- Administer medications and treatments according to provider orders
- Perform nursing assessments and document patient care accurately
- Assist with chronic disease management and follow-up care
- Communicate patient needs and changes in condition to the appropriate healthcare providers
- Educate patients on medications, treatment plans, and healthy practices when appropriate
- Maintain accurate medical records and follow established nursing protocols
- Collaborate with the healthcare team to support continuity and quality of care

Qualifications
- Current, unrestricted LPN license in Alabama
- Current CPR/BLS certification
- Strong clinical assessment, medication administration, and documentation skills
- Ability to work independently while collaborating with the healthcare team
- Compassionate, professional approach to patient care
- Previous correctional, chronic care, or behavioral health experience is helpful but not required
