Position Overview
The Intake Nurse (RN/LPN) plays a critical role in the initiation of home health services. This position is responsible for reviewing referrals, verifying patient eligibility, and coordinating timely and appropriate admission to services. The Intake Nurse serves as a liaison between referral sources, clinicians, and patients to ensure smooth transitions into care while maintaining compliance with clinical and regulatory standards.

- Referral sources (physician offices, hospitals, skilled nursing facilities)
- Insurance case managers
- Patients and families
Minimum Qualifications
- Current RN or LPN license in the state of practice
- Minimum of 2 years nursing experience (home health or related healthcare setting preferred)
- Intake, utilization review, or care coordination experience strongly preferred
- Working knowledge of Medicare, Medicaid, and private insurance guidelines
- Strong communication and interpersonal skills
- Ability to work efficiently in a fast-paced environment
- Proficient with EMR and intake software
- Bilingual preferred
Physical Requirements
Manual Dexterity: Ability to use computer, telephone, fax, and related tools for extended periods
Sitting/Standing: Prolonged sitting with intermittent standing or walking
Lifting: Occasional lifting of supplies or documents up to 20 lbs
Job Responsibilities
- Receive and review home health referrals for completeness and appropriateness
- Verify insurance eligibility, authorizations, and required documentation
- Communicate with referral sources to clarify orders and obtain missing information
- Coordinate with scheduling and field staff to ensure timely admissions
- Ensure physician orders and clinical documentation are complete prior to start of care
- Maintain accurate records in the electronic medical record system
- Educate patients and families on services, expectations, and next steps
- Monitor compliance with CMS regulations and agency policies
- Collaborate with QAPI staff to ensure clinical appropriateness and accurate data entry
- Provide professional customer service representing the agency
- Maintain compliance with HIPAA and all applicable laws
- Perform additional duties as assigned
