Rapid Engagement Specialist
Southeast is seeking an Engagement Specialist to provide clinical and support efforts towards providing crisis intervention counseling, mental health assessments, utilization of agency and community resources and hospital prescreening. This person is responsible for communicating with and de-escalating clients in crisis and providing high quality client care.
Responsibilities:
- Quickly establish contact with individuals referred through emergency departments, crisis units, shelters, outreach teams, or community partners.
- Build rapport and trust with clients who may be hesitant, resistant, or experiencing acute stress.
- Conduct rapid, comprehensive assessments of clients’ medical, mental health, housing, and social needs.
- Identify urgent concerns such as risk of homelessness, untreated mental illness, substance use, or lack of insurance.
- Collaborate with multidisciplinary teams
- Advocate for clients to receive timely services and equitable treatment.
- Monitor outcomes and adjust interventions to improve stability and connection to care.
- Accurately document all interactions, assessments, and outcomes in electronic health or case management systems.
- Track performance metrics such as time to engagement, successful linkages, and client satisfaction.
Requirements:
- Master’s degree and LSW/LPC required.
- Experience with SPMI and/or SUD population preferred.
- Strong interpersonal and communication skills.
- Ability to work effectively as part of an interdisciplinary team.
- Crisis intervention and de-escalation skills.
- Knowledge of community resources and healthcare services.
- Valid driver's license and insurance.
