What are the responsibilities and job description for the PRN Nurse Practitioner position at Southeast Healthcare?
We are a Federally Qualified Health Center (FQHC) committed to providing high-quality, compassionate care to underserved communities in rural Ohio. We are seeking a contingent (PRN) Psychiatric Nurse Practitioner (PMHNP) to join our integrated care team and help address the mental health needs of our diverse patient population.
Responsibilities:
- Perform psychiatric evaluations, diagnostic assessments, and medication management with a primary focus on adults and children with serious mental illnesses.
- Collaborate closely with primary care providers, therapists, case managers, and community agencies to coordinate wraparound services and integrated care plans.
- Support patients through stabilization, crisis prevention, and long-term care management.
- Document care thoroughly in our electronic health record (EHR) system.
- Licensed Psychiatric Mental Health Nurse Practitioner (PMHNP-BC) in Ohio.
- Experience in community mental health, primary care integration, or working with underserved populations is a plus.
- Strong commitment to patient-centered care.
- Strong interpersonal and communication skills.
- Ability to work effectively as part of an interdisciplinary team.
- Crisis intervention and de-escalation skills.
