Role Description This is a PRN, Registered Nurse role based in Albany,GA area. The Registered Nurse will provide direct patient care in the home setting, including assessments, care planning, wound care, medication administration, and monitoring of vital signs and symptoms. The role includes coordinating care with caregivers, physicians, and family members, documenting clinical findings, and updating care plans in accordance with organizational protocols and regulatory standards. The Registered Nurse will offer education to patients and families on disease management, safety, and home care procedures, and will participate in quality improvement efforts to support consistent, high-quality service delivery.
Qualifications
- Current, unrestricted Registered Nurse (RN) license in the state of Ohio and graduation from an accredited nursing program.
- Clinical skills in patient assessment, care planning, wound care, medication administration, and management of chronic and post-acute conditions.
- Strong communication, documentation, and interpersonal skills to collaborate effectively with patients, families, caregivers, and interdisciplinary teams.
- Ability to work independently in home environments, prioritize tasks, and exercise sound clinical judgment while adhering to safety and infection control standards.
- Experience in home health, hospice, geriatrics, or dementia care is highly beneficial; familiarity with electronic health records and basic computer skills preferred.
- Valid driver’s license, reliable transportation for home visits, and willingness to travel within the Greater Cleveland area.
