Duties
- Assess patients’ physical conditions by conducting thorough evaluations of mobility, strength, flexibility, and pain levels.
- Design and execute individualized treatment plans that align with patient needs, medical conditions, and recovery objectives.
- Implement therapeutic interventions such as manual therapy, trigger point therapy, therapeutic exercises, and modalities to promote healing and functional improvement.
- Monitor patient progress regularly, adjusting care plans as necessary to optimize outcomes.
- Document all patient interactions accurately using electronic health records (EHR) systems while adhering to clinical documentation standards.
- Collaborate closely with healthcare professionals—including physicians, nurses, occupational therapists, and social workers—to coordinate effective care strategies.
- Educate patients and their families on treatment routines, home exercises, and community resources to support ongoing recovery and wellness.
Requirements
- Valid licensure as a licensed Physical Therapist in the applicable state or region.
- Proven experience practicing in a physical therapy role within outpatient clinics, inpatient hospitals, or home health settings.
- Strong knowledge of physiology, anatomy, neurology, orthopedics, geriatrics, pediatrics, and related medical terminology.
- Demonstrated ability to develop comprehensive treatment plans tailored to diverse patient populations including geriatrics and pediatric clients.
- Experience with outpatient rehabilitation centers or inpatient hospital environments is highly desirable.
- Proficiency in electronic health records (EHR) management systems such as EMR systems for accurate documentation of patient care activities.
- Excellent client communication skills with a compassionate approach to patient care and education.
- Familiarity with discharge planning processes and working within clinical documentation standards is essential.
- Ability to work collaboratively with healthcare teams to develop effective treatment strategies for acute care and community-based patients.
