Job Responsibilities
- The RCM will lead and supervise the residential team, including nursing staff, caregivers, and support personnel, ensuring high standards of patient care and service delivery.
- Oversee resident admissions, discharges, and ongoing assessments to ensure personalized care plans are implemented effectively.
- Coordinate medical services including critical care, hospital medicine, outpatient clinics, and post-acute care to meet residents’ diverse health needs.
- Leadership includes management of daily operations such as medication administration, vital signs monitoring, discharge planning, and medical records documentation in compliance with HIPAA regulations.
- Ensure safety protocols are followed rigorously by conducting routine inspections, emergency response drills, and maintaining security measures within the residence.
- The RCM will communication between residents, families, healthcare providers, and external agencies to promote transparency and collaborative care.
- Oversee utilization management processes including diagnostic evaluations, case management, intake procedures, and patient assessment workflows.
- Maintain accurate documentation related to medical records in Long Term Care, incident reports, and compliance audits while supporting quality improvement initiatives.
- Implement policies related to infection control, behavioral health support, geriatrics and care strategies.
Qualifications
- Demonstrate leardership abilities
- Proven experience in skilled nursing facility or healthcare facility management with a strong background in patient service and patient care coordination
- Knowledge of infectious disease care protocols and acute care procedures essential for managing complex health conditions
- Experience working within managed care settings or outpatient clinics
- Familiarity with medical terminology, physiology knowledge, discharge planning processes, and medical-surgical practices.
- Prior involvement in inpatient or post-acute care environments with expertise in patient assessment and vital signs monitoring.
- Strong organizational skills with the ability to oversee intake procedures, care coordination, utilization management activities, and diagnostic evaluations.
- Excellent communication skills for collaboration across multidisciplinary teams including and primary care providers.
- Ability to navigate HIPAA regulations diligently while managing medical records and sensitive information.
- Geriatrics experience is a plus.

Pay: $110,000.00 - $120,000.00 per year
