JOB LOCATION : Berrien Campus
DEPARTMENT: BER - GPU
SCHEDULE: Full Time, Weekend Day Shift,
POSITION SUMMARY Works as part of an interdisciplinary team to facilitate the delivery of services to patients/families. Coordinates the discharge planning process. Coordinates individual, family, and group counseling sessions. Uses assessment skills to accurately identify patientsâ needs and demonstrates the ability to provide safe, therapeutic environment for all ages, pediatric, youth, adolescent, adult, and geriatric patients. Participates in patient/family teaching. Uses teaching literature and techniques for different age groups and learning needs. KNOWLEDGE, SKILLS & ABILITIES Education/Licensure/Certification: Master's Degree in Social Work. Licensure LMSW required. LCSW preferred. Previous experience as an LMSW in a hospital setting strongly preferred. Experience with psychiatric patients preferred. Ability to communicate clearly with a diverse population of patients/family units of all ages, variety of cultures, etc. Good organization, prioritization, and problem solving skills. Strong ability to establish and maintain effective working relationships with a variety of personnel at all levels within and external to the organization. Ability to interpret and implement the policies and procedures of SGMC Health, regulatory agenciesâ standards and rules, reimbursement rules, etc. Flexible in work schedule. Computer skills required.
