The purpose of the Case Manager/Instructor position is to support returning citizens as they transition successfully from incarceration back into the community. This position provides comprehensive reentry services focused on reducing recidivism and promoting long-term stability through individualized support, advocacy, education, life skills development, behavioral health connections, housing assistance, family reintegration, and employment readiness services. The Case Manager/Instructor maintains ongoing contact with participants to ensure progress toward individualized reentry goals and provides follow-up support after program completion to encourage continued stability and successful community integration.

Characteristic Duties and Responsibilities

  • Recruits prospective participants, determines eligibility, and administers risk and needs assessments.
  • Provides comprehensive case management services designed to address barriers to successful reentry, including housing instability, behavioral health needs, substance use recovery, education, transportation, family reunification, life skills, and employment readiness.
  • Develops individualized reentry plans that identify participant strengths, barriers, personal goals, support systems, and appropriate interventions and community resources.
  • Assists participants in accessing community resources and evidence-based programs that support successful reintegration and reduce recidivism.
  • Provides guidance and support related to education, life skills, communication skills, financial literacy, conflict resolution, personal responsibility, and healthy decision-making.
  • Facilitates evidence-based classes and interventions identified by the South Central Tennessee Workforce Alliance as appropriate for incarcerated and returning citizens.
  • Coordinates services with correctional facilities, treatment providers, housing agencies, educational institutions, workforce partners, mental health providers, and other community organizations.
  • Conducts interviews and assessments to identify psychosocial needs and barriers to community reintegration.
  • Provides support and referrals related to mental health treatment, addiction recovery services, healthcare access, social services, and family/community support systems.
  • Assists participants in preparing for independent living and long-term self-sufficiency.
  • Supports participants with employment readiness activities including career exploration, job readiness, résumé development, and connections to employers when appropriate.
  • Monitors participant progress and maintains regular contact to provide encouragement, accountability, crisis intervention, and ongoing support services.
  • Documents participant progress, case notes, referrals, and follow-up activities within the automated case management system in accordance with agency policies and funding requirements.
  • Completes required reports and documentation in a timely manner.
  • Works collaboratively as part of a multidisciplinary team to achieve program performance goals and positive participant outcomes.
  • Maintains compliance with policies and procedures of the South Central Tennessee Workforce Alliance as well as applicable federal, state, and local regulations.
  • Performs other duties as assigned.

Education, Training and Experience Required

  • Bachelor’s degree in psychology, social work, counseling, criminal justice, sociology, human services, business administration, or related field.
  • One year of experience in case management, reentry services, workforce development, behavioral health services, corrections, social services, or related human services field preferred.
  • Experience working with justice-involved individuals or underserved populations preferred.

Knowledge, Skills, Abilities; Worker Characteristics

  • Knowledge of trauma-informed care, motivational interviewing, addiction and recovery resources, crisis intervention, behavioral health resources, and community support systems.
  • Knowledge of reentry challenges and barriers faced by justice-involved individuals.
  • Ability to establish professional relationships and maintain appropriate boundaries with participants from diverse backgrounds.
  • Strong interpersonal, communication, problem-solving, and conflict-resolution skills.
  • Ability to facilitate groups, workshops, and evidence-based interventions.
  • Ability to communicate effectively through active listening, guidance, encouragement, and constructive feedback.
  • Ability to function effectively as part of a collaborative team.
  • Strong organizational and time management skills with the ability to manage large caseloads and maintain accurate documentation.
  • Ability to utilize computer software applications including Word, Excel, case management systems, and internet-based resources.
  • Knowledge of community resources and referral systems available to support participant reentry and stabilization.
  • Supervisory experience preferred.

Supplemental Functions

  • Attends training workshops, seminars, and conferences as assigned by supervisor.

Working Conditions

  • Flexible working hours (Normal work hours are 8:00 a.m. – 4:30 p.m., but this position may require some evening and Saturday work hours).
  • Normal sitting and movement in an office environment. May require occasional lifting up to 25 pounds, bending, and moving furniture.
  • Requires coordination with community service agencies and institutional partners.
  • Requires travel throughout the South Central Tennessee Workforce Alliance service area for meetings, participant services, and other job-related activities.
  • May require occasional overnight travel for training and professional development.
  • Frequent computer and office equipment use.

Pay: $45,000.00 - $55,000.00 per year
