Position Overview:
We are seeking a dedicated and compassionate Therapist to join our team. This position operates within a Certified Community Behavioral Health Clinic (CCBHC) model, providing person-centered, recovery-focused, and trauma-informed mental health services to individuals in the community. In this role, you will provide direct clinical services, crisis intervention, and conflict resolution while collaborating with multidisciplinary teams and community partners to promote stabilization, recovery, and continuity of care.
Qualifications:
- Must have a Master’s Degree (MS) in psychology, social work, counseling, or another behavioral health-related field
- Strong clinical assessment, communication, crisis intervention, and documentation skills
- Ability to work independently and collaboratively as part of a multidisciplinary treatment team
- Knowledge of person-centered treatment planning, trauma-informed care, and evidence-based behavioral health practices preferred
- Licensed or license-eligible preferred
Responsibilities:
- Provide direct mental health services, therapeutic interventions, crisis intervention, and conflict resolution services to consumers in the community
- Conduct clinical assessments and assist in developing individualized, person-centered treatment plans based on consumer needs and recovery goals
- Provide evidence-based, trauma-informed, and recovery-focused therapeutic interventions
- Offer consultation and educational services to community agencies, professionals, families, and the general public
- Collaborate with therapists, psychiatric providers, nurses, care coordinators, peer support specialists, and other professionals to provide coordinated, integrated care within the CCBHC care model
- Support appropriate referrals, transitions of care, and connections to behavioral health and community-based resources
- Oversee, maintain, and complete designated documentation in consumer records in accordance with agency standards, regulatory requirements, and CCBHC guidelines
- Maintain accurate, timely, and confidential documentation of services, treatment progress, and clinical interventions
- Promote recovery, resilience, independence, and continuity of care for individuals receiving behavioral health services
Work Environment:
Provide behavioral health services in community-based and other appropriate settings based on consumer needs. Work collaboratively with multidisciplinary staff and community partners to support accessible, coordinated, and recovery-focused behavioral health care.
