Position Overview:

We are seeking a dedicated and compassionate Children’s Low Intensity Care Coordinator to join our team serving Butler and Crenshaw Counties. This position operates within a Certified Community Behavioral Health Clinic (CCBHC) model, focusing on person-centered care coordination, family engagement, early intervention, and connecting children and families to behavioral health and community-based services. The ideal candidate will be flexible, professional, and collaborative, with a passion for supporting children and families in achieving stability, wellness, and access to appropriate care.

Qualifications:

  • Bachelor’s Degree required
  • Strong communication, documentation, and organizational skills
  • Ability to work effectively with children, families, schools, and community partners
  • Ability to work independently while collaborating as part of a multidisciplinary team
  • Knowledge of child behavioral health services, family systems, and community resources preferred
  • Professional, flexible, and team-oriented approach to service delivery

Responsibilities:

  • Schedule and conduct home visits with children and families to assess needs and support service engagement
  • Gather relevant clinical, social, and environmental information to support individualized care planning
  • Develop and maintain person-centered plans of care focused on the child’s strengths, needs, and treatment goals
  • Establish and maintain collaborative relationships with schools, healthcare providers, social service agencies, and community partners
  • Assist children and families in accessing behavioral health treatment, community resources, educational supports, and other entitled services
  • Support continuity of care and coordination between providers to ensure comprehensive, family-centered service delivery within the CCBHC care model
  • Advocate for children and families while helping reduce barriers to treatment and community support services
  • Maintain accurate, timely, and confidential documentation in compliance with agency standards and CCBHC requirements

ALL candidates must be able to type and have basic computer skills, possess a valid driver’s license, eligibility for coverage under center liability insurance and individually held vehicle liability insurance with a clear background check.
